{"id":61,"date":"2015-01-08T19:49:23","date_gmt":"2015-01-09T02:49:23","guid":{"rendered":"http:\/\/www.theherbalista.com\/?p=61"},"modified":"2015-10-12T17:20:48","modified_gmt":"2015-10-13T00:20:48","slug":"how-to-get-a-medical-marijuana-mmj-card-in-az","status":"publish","type":"post","link":"https:\/\/www.theherbalista.com\/index.php\/2015\/01\/08\/how-to-get-a-medical-marijuana-mmj-card-in-az\/","title":{"rendered":"HOW TO GET A MEDICAL MARIJUANA (MMJ) CARD IN AZ"},"content":{"rendered":"<header><a href=\"http:\/\/www.theherbalista.com\/wp-content\/uploads\/2015\/10\/wpid-wp-1444676650111.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-124\" src=\"http:\/\/www.theherbalista.com\/wp-content\/uploads\/2015\/10\/wpid-wp-1444676650111-595x595.jpg\" alt=\"wpid-wp-1444676650111.jpg\" width=\"321\" height=\"321\" srcset=\"https:\/\/www.theherbalista.com\/wp-content\/uploads\/2015\/10\/wpid-wp-1444676650111-595x595.jpg 595w, https:\/\/www.theherbalista.com\/wp-content\/uploads\/2015\/10\/wpid-wp-1444676650111.jpg 640w\" sizes=\"auto, (max-width: 321px) 100vw, 321px\" \/><\/a>The most popular question I am contacted about is how to go about getting a medical marijuana card in Arizona.\u00a0Here is the process I have used as a <a href=\"http:\/\/www.azdhs.gov\/medicalmarijuana\/patients\/adult.htm\" target=\"_blank\">patient<\/a>: (if you are under the age of 18, there is a separate process\u2013 feel free to connect with me if you would like details\/assistance).<strong>Schedule Appointment<\/strong> \u2013\u00a0A Medical Marijuana Physician Certificationis required\u00a0for your application.\u00a0This can be signed by any doctor (MD, ND, DO licensed to practice medicine in AZ) that can\u00a0certify your qualifying condition:<\/p>\n<blockquote><p>Cancer\u00a0Glaucoma,\u00a0Human Immunodeficiency Virus (HIV),\u00a0Acquired Immune Deficiency Syndrome (AIDS), \u00a0Hepatitis C,\u00a0Amyotrophic Lateral Sclerosis (ALS),\u00a0Crohn\u2019s disease,\u00a0Agitation of Alzheimer\u2019s disease<\/p><\/blockquote>\n<p><em>A chronic or debilitating disease or medical condition or the treatment for a chronic or debilitating disease or medical condition that causes:<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>Cachexia or wasting syndrome;<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>Severe and chronic pain;<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>Severe nausea;<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>Seizures, including those characteristic of epilepsy;<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>Severe or persistent muscle spasms, including those characteristic of multiple sclerosis<\/em><\/p>\n<p style=\"padding-left: 30px;\"><em>PTSD, Post Traumatic Stress Disorder<\/em><\/p>\n<p>If you have a personal physician that you are currently working with that will sign your attestation, you would contact them for an appointment and pay according to your typical arrangement. If you are not currently working with a physician or working with one who is not supportive of medical marijuana, there are specialty clinics that will certify your qualifying condition for a fee that can be upward of $150 for the visit. Find one most convenient for you as they are all about the same in offerings\/rates. What is it exactly\u00a0are you paying them for?<\/p>\n<p>The law and rules specify requirements for issuing written certifications for patients for the medical use of marijuana. A physician is required to:<\/p>\n<ul>\n<li>Have made or confirmed a diagnosis of a debilitating medical condition, as defined in <a href=\"http:\/\/www.azleg.gov\/search\/oop\/qfullhit.asp?CiWebHitsFile=\/ars\/36\/02801.htm&amp;CiRestriction=36%2D2801\" target=\"_blank\">A.R.S. \u00a7 36-2801<\/a>, for the qualifying patient; Have established a medical record for the qualifying patient and am maintaining the qualifying patient\u2019s medical record as required in <a href=\"http:\/\/www.azleg.gov\/search\/oop\/qfullhit.asp?CiWebHitsFile=\/ars\/12\/02297.htm&amp;CiRestriction=12%2D2297\" target=\"_blank\">A.R.S. \u00a7 12-2297<\/a>;<\/li>\n<li>Have conducted an in-person physical examination of the qualifying patient within the last 90 calendar days appropriate to the qualifying patient\u2019s presenting symptoms and the debilitating medical condition the physician diagnosed or confirmed;<\/li>\n<li>Have reviewed the qualifying patient\u2019s medical records, including medical records from other treating physicians from the previous 12 months, the qualifying patient\u2019s responses to conventional medications and medical therapies, and the qualifying patient\u2019s profile on the Arizona Board of Pharmacy Controlled Substances Prescription Monitoring Program database;<\/li>\n<li>Have explained the potential risks and benefits of the medical use of marijuana to the qualifying patient, or if applicable, the qualifying patient\u2019s custodial parent or legal guardian;<\/li>\n<li>If the physician has referred the qualifying patient to a dispensary, have disclosed to the qualifying patient, or if applicable, the qualifying patient\u2019s custodial parent or legal guardian, any personal or professional relationship I have with the dispensary; and<\/li>\n<li>Attest that, in the physician\u2019s professional opinion, the qualifying patient is likely to receive therapeutic or palliative benefit from the qualifying patient\u2019s medical use of marijuana to treat or alleviate the qualifying patient\u2019s debilitating medical condition.<\/li>\n<\/ul>\n<p>This is just a fancy way of saying that the doctor is going to make sure that you\u2019re medically in need of marijuana and qualify based on the above. Some of these places\u00a0also offer to complete your paperwork to the state for you for an additional fee, though it is something you can save yourself some money by doing yourself if you feel comfortable technologically submitting documents online. Once you have this form signed from your physician, you\u2019re all set to complete the rest of your items on your own.<\/p>\n<p><strong>Fill Out, Sign, Upload, Pay.<\/strong><\/p>\n<p>It is that simple. Fill out your\u00a0Patient Attestation\u00a0form, sign and date it. Upload this document with your Physician\u2019s signed &amp; dated form, in addition to a passport style photo, state identification, and additional info with your online application.
